Telehealth Consent

This Telehealth Informed Consent (“Consent”) describes the nature, benefits, risks, and alternatives of receiving health care services remotely through IAM’s telehealth platform (“IAM,” “we,” “us,” or “our”). IAM is a technology platform that connects you with independent Medical Group(s) and licensed Provider(s). By proceeding to use IAM’s telehealth services, you acknowledge that you have read, understood, and agree to this Consent, IAM’s Terms and Conditions, and IAM’s Privacy Policy.

1. What Is Telehealth

Telehealth is the delivery of health care services using secure electronic communication technologies, allowing a Provider to evaluate, diagnose, treat, and advise you remotely through video, audio, and messaging.

2. IAM’s Role

IAM provides the technology platform that facilitates telehealth. IAM does not practice medicine, provide diagnoses, or make treatment decisions. All clinical care, including diagnosis and treatment, is provided solely by independent Provider(s) affiliated with Medical Group(s).

3. Anticipated Benefits

  • Convenient access to health care from a remote location.
  • Reduced travel time and scheduling flexibility.
  • Useful for consultations, follow-up visits, medication management, and certain health concerns.

4. Risks & Limitations

You understand and acknowledge that telehealth has inherent risks and limitations, including:

  • Technical failures, including connection loss, audio/video disruptions, and equipment malfunction.
  • Potential risks to the privacy and security of electronic communications.
  • Limitations in the ability to perform a physical examination or certain diagnostic tests.
  • The possibility that the information transmitted may not be sufficient for an accurate assessment, diagnosis, or treatment.
  • Delays in care, testing, or prescription fulfillment.

5. No Guarantee of Diagnosis or Prescription

You understand that there is no guarantee that a diagnosis will be made, that a treatment or prescription will be issued, or that any specific outcome will result from your consultation. Provider(s) may decline to treat, prescribe, or continue treatment when it is not medically appropriate.

6. Emergencies

IAM and telehealth services are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or seek in-person emergency care immediately.

7. Privacy & HIPAA

Your privacy and the confidentiality of your health information are important. Your Protected Health Information (PHI) is handled in accordance with HIPAA as described in IAM’s Privacy Policy. While we use safeguards to protect information, no electronic transmission or storage method is completely secure.

8. Alternatives to Telehealth

You may choose to receive health care services through in-person visits with a health care provider instead of telehealth. You are free to discuss alternatives with your Provider.

9. Labs, Pharmacy & Prescription Management

Through the Services, your Provider may order laboratory tests from independent laboratories, including Labcorp and, occasionally, Quest Diagnostics (“Labs”), and prescriptions may be dispensed by licensed pharmacies, including 1st Choice, and transferred between Pharmacies as needed for fulfillment. You understand that prescription availability, timing, and delivery are subject to the policies of the applicable Pharmacy.

10. Your Responsibilities

You agree to:

  • Provide accurate, complete, and current health information.
  • Maintain the confidentiality of your account credentials.
  • Use the telehealth services only for lawful and personal purposes.
  • Follow the medical advice and instructions of your Provider(s).
  • Contact emergency services in an emergency.

11. Consent & Acknowledgement

By proceeding to use IAM’s telehealth services, you acknowledge and agree that:

  • You are 18 years of age or older.
  • You have read, understood, and voluntarily agree to this Consent.
  • You understand the benefits, risks, and limitations of telehealth described above.
  • You consent to receiving health care services via telehealth and to the electronic transmission of your information as necessary to provide care.
  • You agree to IAM’s Terms and Conditions and Privacy Policy.

13. Governing Law

This Consent is governed by the laws of the State of Delaware, without regard to conflict-of-laws principles.
