Submitting Claims to Insurance, FSA, or HSA

At IAM Clinic, we are a self-pay clinic. That means we do not bill insurance, are not in-network, and do not work with insurance companies directly.

However, you may be able to submit your own claim for reimbursement through your insurance provider, FSA (Flexible Spending Account), or HSA (Health Savings Account). Many of our patients do this successfully—but outcomes vary.

What We Provide

We’ll give you a Superbill or itemized receipt that includes:

  • Provider name, NPI, and clinic address
  • Date(s) of service
  • CPT (procedure) codes
  • ICD-10 (diagnosis) codes
  • Amount paid

This applies to:

  • Telemedicine consultations
  • Prescription medications
  • Lab testing (when ordered through IAM)
  • Supplements (if medically recommended)

Important Notes

  • IAM Clinic does not contact insurance on your behalf.
  • We cannot verify what is covered or reimbursed—that’s up to your insurance company, FSA, or HSA provider.
  • Some items (like supplements or weight loss medications) may require a Letter of Medical Necessity for reimbursement.
    Please note: IAM Clinic does not issue Letters of Medical Necessity.
  • Reimbursement is never guaranteed. Even with proper documentation, approval is at the discretion of your benefits administrator.
